Shadday Cooper is our Spanish speaking Pathways HUB Community Health Worker for East Williamson County. She is currently helping pregnant mothers up to 12 months postpartum.
About Pathways
COMMUNITY HUB SERVICES FOR NEW FAMILIES
In the Pathways program, our Community Health Workers look at everything a family needs to stay healthy, including access to housing, healthy food, medical care, health insurance, safety, transportation, education, employment, and other services. We provide home and community-based services based on individual needs and work alongside family members to help lower health risks and see positive results.
