Community HUB Services for new families
About Pathways
In the Pathways program, our Community Health Workers look at everything a family needs to stay healthy, including access to housing, healthy food, medical care, health insurance, safety, transportation, education, employment, and other services. We provide home and community-based services based on individual needs and work alongside family members to help lower health risks and see positive results.

Who is eligible?
Pregnant and new moms with babies up to one year old who are uninsured or enrolled in Medicaid
Where is this available?
Currently serving Eastern Williamson County including Taylor, Bartlett, Granger, Coupland, Hutto, and Thrall
What services are offered?
Assistance with basic needs referrals and access to health care
Health education
Care planning and ongoing support
Services are provided in English and Spanish, and additional languages are available through translation services.
Each mom who completes their first visit will receive a $50 gift card. Apply Now
